Athletics Sign-up

At this time, only 7th and 8th grade students can register. It is possible there will be some form of
athletics programming available for 6th grade students, but we are still in the planning phase for
this. Stay tuned!

How do I sign up?

  1. Go to: shoreline-wa.finalforms.com 
  2. Locate the parent/guardian icon and click NEW ACCOUNT below.
  3. Type YOUR NAME, DATE OF BIRTH, and EMAIL. Next, click REGISTER. (NOTE: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, then check your spam folder. If you still can not locate the FinalForms email, then email support@finalforms.com informing our team of the issue.)
  4. Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman. Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
  5. Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
  6. Click REGISTER STUDENT for your first child.

What information will I need to register a student?

  •  Sports Physical and health information.

Sports Physicals are usually good for 2 years – make sure it is completed and signed by your physician at the time of your athletes visit to the doctor.  *Prior KMS athletes who have a current physical on file, please check with the Athletic secretary to make sure it is still up to date!

Shoreline School District Sports Physical Form

  •  Insurance company and policy number.
  • Doctor, dentist, and medical specialist contact information. Hospital preference and contact information.

Practice Days

Practices run every day after school and will end at 5:00 (Practices are closed to friends and family). Your student can ride the activity bus home after practice on Tuesday, Wednesday and Thursday.

On early release Wednesdays, students are allowed to stay at school until practice begins at 3:15 if needed. They are also allowed to go home as long as they are back for practice at 3:15.

  • Home Games: Please arrange to transport your athlete home after matches.
  • Away Games: Teams will be transported by bus to and from away matches. Students returning on the bus will call parents from the bus, within 15 minutes of arrival to Kellogg.
  • The only exception to roundtrip transportation is when we play our inter-district game
    versus Einstein Middle school. Teams will be transported to EMS but athletes will need
    to arrange their own rides home.

Uniforms/Player Fee

All players will be issued a team uniform. Please ensure that these items are cleaned regularly and returned in the best-possible condition at the end of the season.  They will be returned when the season is over.

  • A $30 ASB and a $100 sports fee is due before the first competition day. Please make this payment online via InTouch or in the Kellogg Office.

  • Scholarships are available for students who need financial assistance. Please contact a school counselor.
  •  No refunds will be approved for students who are dropped for athletic code violations or who quit after the first interscholastic contest.
